Your Quartz Premium Invoices

    New Quartz InvoiceWe have updated our health insurance premium invoices.

    The new layout of the invoice features the Quartz logo and a scannable barcode that will help us process your payments more quickly and accurately.

    Please return the bottom portion of your invoice with each payment (also known as a “remit”). Please review the remit as the information, because the “make check payable to” has changed too.
