New Member Drug Supply*

    If you’re new to Quartz (a member for less than 90 days), you may be taking medicines that require a Prior Authorization for coverage.


    Just call Pharmacy Services at (800) 788-2949 to ask for it. If approved, Quartz will authorize coverage for these drugs for up to 90 days (in 30-day increments at the usual copayment).

    After those 90 days, prior authorization will be required for additional coverage. In the meantime, you can discuss your options with the doctor who prescribed the medicine. The complete formulary, a description of your plan’s diŽerent pharmacy benefit designs and copayment or coinsurance requirements and current prior authorization criteria are available at or in writing upon request or by calling
    (888) 450-4884.


    Please note: This policy only applies to Quartz-branded plans underwritten by Unity and medications that a member was taking prior to joining Quartz coverage being eŽeffective. Standard copays based on the member’s drug benefit and formulary status of the medication apply, depending on your plan.

    * The State of Wisconsin and Local Government participants receive their pharmacy benefits through Navitus. This information applies to those members that have pharmacy benefits with Quartz. Refer to MyChart to see your plan’s prescription benefits coverage.
