A New Wellness Program that Points to a Healthier You
As we get closer to the launch of Quartz Well in January 2018, we are sharing more details of this personalized digital wellness program that was designed with your health and wellness goals in mind.
Healthy choices made simple
Quartz Well is an easy-to-use program you can access through your MyChart account. Quartz Well encourages healthy choices by offering points for a variety of activities and services. From your annual physical to health and wellness classes, you can earn points for preventive health care, fitness activities and health engagement.
Releasing Health Information
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) generally requires that we obtain an authorization to release Protected Health Information (PHI) to your family or a friend (some exceptions may apply). If you would like us to discuss your information with someone other than yourself, please fill out the Authorization for Disclosure of Protected Health Information form, which is available at QuartzBenefits.com/phiform or in QuartzMyChart. You can specify which information may be released to named people or entities. You may authorize anyone, including a friend, a family member, or an employer or potential employer. Once completed, you can return it to Quartz and an authorization will be entered to allow release of the requested information.
As a requirement of the Affordable Care Act, your 2017 federal tax return will ask if you and your dependents had qualifying health care coverage in 2017. The 1095-B Form will detail your 2017 coverage and should be kept with your tax records.
Let’s face it, this time of year we all feel like the Griswold family with big plans for the holidays. Although a holiday movie classic can get you in the spirit of the season, it can also show what could go wrong if you don’t take the necessary steps to stay safe.
Here’s how to keep you and your family — pets included — out of harm’s way –
- Step up carefully when decorating. Use a step stool or ladder to place decorations in high places.
- Be cautious with candles. Always keep them at least 12 inches away from anything that can burn, on a stable surface and out of reach of children and pets. Don’t forget to blow them out when you leave the room.
- Check holiday lights. Take a close look to find any exposed or frayed wires, loose connections or broken sockets.
- Trim trees with kids and pets in mind. Keep breakable ornaments or those with metal hooks
near the top of the tree — and away from curious little ones and animals.
- Fireproof your tree too. Live trees dry out rapidly. Try to keep the stand filled with water and turn off all tree lights and decorations when not in use. Keeping your tree away from fireplaces, radiators and portable heaters will also help.
