1095 Forms for 2017

    Contact Preferences in MyChartAs a requirement of the Affordable Care Act, your 2017 federal tax return will ask if you and your dependents had qualifying health care coverage in 2017. The 1095-B Form will detail your 2017 coverage and should be kept with your tax records.

    In January, we will send you a printed copy of your 1095-B Form* and it will also be available in MyChart. If you don’t want to receive a printed copy in the mail, please update your preferences in MyChart by January 1, 2018. We will email you when the form is available to print from MyChart.

    * Note: BadgerCare Plus, Health Insurance Marketplace (HIM) and Medicare Select members may receive a similar form from Medicaid, HIM or Medicare. They will not receive a 1095-B form from Quartz.
