    Case Management Can Help You When You Need It the Most

    Have you recently experienced a significant event or been diagnosed with a complex or chronic condition? Did you know we offer case management services to our members depending on your plan’s benefits?

    WHAT IS CASE MANAGEMENT?

    Case management is the coordination of care, services and resources for members with certain complex or chronic conditions. There is no extra cost for this service to health plan members with this benefit, and you can refer yourself.

    Case management staff works closely with you and your caregivers to guide you through medical and community services that can be difficult to navigate. They coordinate health services and resources to make sure that you get the best care possible. The case management staff can assist you in setting goals and creating a plan that will maximize your health and improve quality of life.

    Case Management Staff May Help

    • Provide education about your health condition and care
    • Support and reinforce treatments and therapies recommended by your health care provider
    • Coordinate health services and clarify insurance benefits
    • Connect you to community services or programs

    Who Might Benefit?

    • Members who have more than one serious health problem such as diabetes, heart failure or COPD
    • Members who have experienced a significant event or diagnosis like cancer, stroke or traumatic brain injury
    • Members who have had several emergency room visits or hospital stays

    An Epidemic Fueled by Opioid Pain Medications

    It’s tough to live with chronic pain, but millions of Americans do. That’s one reason why prescription opioid pain medications are so popular.

    Opioid medications are good at relieving pain in the short term. They also help people with active cancer and people receiving hospice or palliative care cope with pain. While these medications can be safe when taken as prescribed by a health care provider, opioids come with some serious risks, including the risk of addiction and unintentional overdose and death.

    What You Need to Know About Opiods

    • Overdosing on opioids can cause extreme sleepiness, difficulty waking up and stopped breathing. If you suspect someone may have overdosed, call 911 immediately.

    • A nose spray (Narcan®) can quickly reverse an opioid overdose. You don’t need a prescription in most pharmacies.

    • Well-known painkillers, like Vicodin (hydrocodone) and OxyContin (oxycodone), are opioids. So are fentanyl, morphine, codeine and methadone.
